Terms & Conditions
Last Updated: 10/07/2024
Welcome to the Carolina Trauma Recovery and Psychological Services, PLLC website (“Site”). By using this Site and engaging in telehealth services with Dr. Cierra Listermann, you agree to the following Terms and Conditions. Please read them carefully.
1. Services Provided
Carolina Trauma Recovery and Psychological Services, PLLC, operated by Dr. Cierra Listermann, provides individual telehealth therapy services to adults (18+) who are residents of North Carolina. Telehealth services via video conferencing are conducted using Simple Practice and phone sessions are conducted using the business version of Google Voice. Both are secure and HIPAA-compliant platforms, and all services are subject to state and federal laws governing telemedicine.
2. Eligibility
You must be 18 years of age or older, reside in North Carolina, and be physically located in North Carolina at the time of service to engage in therapy sessions provided by Carolina Trauma Recovery. Therapy services are limited to those who meet these criteria, and exceptions cannot be made due to licensure restrictions.
3. Appointments & Cancellations
Appointments are scheduled on a regular basis, typically weekly, with some biweekly appointments available for long-term clients nearing the end of their treatment. In order to respect Dr. Listermann’s limited availability and maintain the integrity of the therapeutic process, clients must give at least 24 hours' notice prior to the scheduled appointment time for cancellations (e.g., if the appointment is scheduled for Tuesday at 4pm, cancellations need to be made prior to 4pm on Monday at the latest). Failure to cancel within this window will result in the full session fee being charged.
If you exceed more than two late cancellations or no shows consecutively, a stricter cancellation policy may be applied. If lack of consistency in attendance to appointments continues it may result in potential termination of services and referral to another provider. This policy is in place to ensure fairness to other clients and those on the waitlist.
4. Payment & Fees
Carolina Trauma Recovery strives to make payment for services as convenient as possible. To accommodate different payment preferences, the following are accepted forms of payment:
Credit Cards: We accept all major credit cards (Visa, MasterCard, American Express, Discover). Payment is processed securely through our billing system at the beginning of each session.
Health Savings Accounts (HSA): HSA cards are accepted for payment of therapy services, provided that therapy is deemed a qualified medical expense under your HSA plan. Please check with your HSA provider if you have any questions about eligibility.
Flexible Spending Accounts (FSA): FSA cards may be used to pay for therapy services, as long as the therapy is an approved medical expense. Clients are encouraged to confirm with their FSA provider to ensure therapy qualifies.
Payment for services is required at the time of your scheduled appointment. If a payment is not made, your appointment may be subject to cancellation. All fees are outlined prior to your first session, and clients are responsible for keeping up-to-date payment information on file.
In the event that a payment is declined, the client will be notified immediately and an alternative form of payment will be requested. Unpaid balances may result in a suspension of services until the balance is settled. Payments made via credit card, HSA, or FSA are non-refundable (unless there is a clerical error).
